- Download, print and complete the Authorization Form.
- Complete all areas. Be sure to specify the dates of service and type of information needed (for example: Discharge summary from 1/10/09 visit).
- Place the completed authorization form in an envelope and mail to Medical Records at Karmanos.

There is a fee for copies of your medical record. Please call the Medical Records Department at (313) 576-9393 for current fee information.
