Malignancies of the Gastrointestinal Tract

Cancers that form in the pancreas are one of two types: exocrine or neuroendocrine tumors (PNETs). Exocrine tumors usually are adenocarcinoma, acinar cell carcinoma, intraductal papillary-mucinous neoplasm (IPMN) and mucinous cystic neoplasm with an invasive adenocarcinoma. Neuroendocrine tumors of the pancreas usually include gastrinoma (gastrin), glucaganoma (glucagon), insulinoma (insulin), somatostatinoma (somatostatin), VIPoma (vasoactive intestinal peptide) and nonfuctional islet cell tumor (no hormones).

Cancers that start in the gallbladder are usually adenocarcinomas, such as papillary adenocarcinoma (papillary cancer). Adenosquamous carcinomas, squamous cell carcinomas and carcinosarcomas may also start in the gallbladder.Cancers that form in the pancreas are one of two types: exocrine or neuroendocrine tumors (PNETs). Also known as bile duct cancer. Many are cholangiocarcinomas. There are three types of bile duct cancers: Intrahepatic bile duct cancers, perihilar bile duct cancers (hilar bile duct cancers), and distal bile duct cancers. According to the American Cancer Society, intrahepatic bile duct cancers form in the smaller bile duct branches of the liver. Perihilar bile duct cancers are also called klatskin tumors and form at the hilum. Distal bile duct cancers are called extrahepatic bile duct cancers. They start outside of the liver, near the small intestine.

Skin Cancers

Below are signs and symptoms that skin cancer might occur. Should you notice any of the following, see your health care provider. It is best to catch skin cancer as early as possible.

Unusual moles, sores, lumps, blemishes, markings, or changes in the way an area of the skin looks or feels
Rough or scaly red patches, which might crust or bleed
Raised growths or lumps, sometimes with a lower area in the center
Open sores (which may have oozing or crusted areas) that do not heal, or that heal and then come back
Wart-like growths
Dark or pigmented skin lesions with irregular edges or color change

Melanoma is less common but is considered one of the most dangerous types of skin cancers because it is more likely to spread to lymph nodes and other organs of the body if it is not detected and treated in the early stages. This cancer forms from the overproduction of melanocytes, the skin and eye cells that produce melanin or skin pigmentation. Melanomas may appear anywhere on the skin, even in areas that are not exposed by the sun. The most common forms of melanoma appear pink, tan, white, brown, or black.

Another less common but considered one of the most dangerous of skin cancers is merkel cell cancer. This is due to the cancer’s ability to spread to other parts of the body. Merkel cell cancer is usually found on the face, but it can also be found on other areas of the body that are exposed to the sun, like the neck and arms. Signs and symptoms of merkel cell cancer are lumps or bumps that are usually pink, red, or purple in appearance.

Soft Tissue and Endocrine Malignancies

This cancer begins in the thyroid gland. There are three main types: differentiated, medullary and anaplastic. Most are differentiated, which include papillary cancer (papillary carcinomas or papillary adenocarcinomas), follicular cancer (follicular carcinoma or follicular adenocarcinoma), and Hurthle cell cancer (oxyphill cell carcinoma). Medullary thyroid carcinoma (MTC) can be sporadic MTC and familial MTC. Anaplastic thyroid cancer is also called anaplastic carcinoma, or undifferentiated carcinoma. There are also thyroid lymphomas and thyroid sarcomas.

Dr. Oyasiji utilizes a minimally invasive approach, ranging from laparoscopic to robotic surgery, as well as regional therapies:

Regional chemotherapy can sometimes help to extend and improve quality of life for some patients with advanced cancers, even if surgery and traditional chemotherapy are not a treatment option for the patient. Dr. Oyasiji specializes in Hyperthermic Intraperitoneal Chemotherapy (HIPEC), Isolated Limb Infusion/Isolated Limb Perfusion (ILI/ILP), Radiofrequency Ablation (RFA), Microwave Ablation (MWA), Transarterial Chemoembolization (TACE) and Yttrium Radioembolization (Y-90).

Ablative Treatments, like RFA and MWA, use heat or cold to destroy and ablate tumors without the need for extensive surgeries. Ablative treatments are delivered directly to the tumor site(s) through special probes. Using ultrasound imaging, the surgical oncologists guides the probes to the correct position and monitors the progress of treatment. In addition to treating cancer, ablative therapies have advantages, including:
- Minimal pain
- Shorter recovery time than traditional surgery or radiation therapy
- Can be used in conjunction with other cancer treatments
- Can be easily repeated if necessary
